Notification of Patient Visitation Rights

The Department of Health and Human Services and the Centers for Medicare and Medicaid Services have issued a final rule effective January 18, 2011 stating that all hospitals are required to inform all patients of their patient rights and responsibilities, including their patient visitation rights, specifying any clinical restriction or limitation of these rights prior to the provision of services when possible.

We would like to take the time to inform you of your patient visitation rights at Horn Memorial Hospital.

Patient Visitation Rights

While receiving care from Horn Memorial Hospital you have the right to receive visitors of your choice subject to your consent including, but not limited to, the following: a spouse, a domestic partner (including a same-sex domestic partner), any other family member, or friend. You have the right to revoke your consent to visitors at any time during your stay.

Horn Memorial Hospital will ensure to the best of our abilities, that all visitors; designated by you, enjoy visitation privileges that are as unrestricted as those your immediate family members would enjoy.

Horn Memorial Hospital will not restrict, limit, or otherwise deny your visitation privileges based on race, color, national origin, veteran status, religion, sex, gender identity, sexual orientation, or disability.

Horn Memorial Hospital retains the right to restrict visitors when clinically necessary.

The following is a list of certain circumstances when a visitor may be restricted:

when the nurse caring for you or the Infection Prevention Team believes the visitor to be infectious
when it is necessary to maintain your personal privacy
when sterile procedures are performed and sterile fields must be maintained
when the visitor is at risk of being exposed to an infectious disease by the patient
when the visitor's presence poses a direct threat to the patient
