Community Health Needs
Ida, Sac and Monona Counties in Iowa
Creating healthy communities requires a high level of mutual understanding and collaboration with the community individuals and partner groups. The development of this assessment brings together community health leaders and providers along with local residents to research and prioritize county health needs as well as to document community health delivery success.
This health assessment will serve as the foundation for community health improvement efforts for the next three years.
Adoption of 2022 Implementation Plan:
The Board of Trustees met on September 26, 2022 where they reviewed and approved the implementation plan, thereby formally adopting the CHNA Implementation Plan. The 2022 CHNA Report was made widely available to the public September 28, 2022 both upon request and on the website at the link below.
Adoption of 2019 Implementation Plan:
A small committee met on September 3, 2019 and developed the CHNA Implementation Plan. The Board of Trustees met on September 30, 2019 where they reviewed and approved the implementation plan, thereby formally adopting the CHNA Implementation Plan. The 2019 CHNA Report was made widely available to the public October 7, 2019, both upon request and on the website at the link below.
Adoption of 2016 Implementation Plan:
Community members and staff met on September 29, 2016 and developed the CHNA Implementation Plan. The 2016 CHNA Report was made widely available to the public October 5, 2016, both upon request and on the website. Following these steps, the Board of Trustees met on November 21, 2016. The Board reviewed the implementation plan and approved the plan, thereby formally adopting the CHNA Implementation Plan. The CHNA Implementation Plan was at that time widely available to the public upon request, and it is available at the link below.
